The Lacamas Creek Mission 



At Lacamas Creek Post Acute, our mission is to be a skilled nursing and rehabilitation center that provides comfort, support and care in a luxurious and home-like setting. As a rehabilitation center, we focus on outcomes and your quality of life, supported by in-house therapy services including physical therapy, occupational therapy, speech therapy, post-op care and pain management, and physiatry services. 


Our skilled nurses, therapists, caregivers, and support staff work together to provide the highest quality of care to every resident. From memory care needs to short-term rehab, we tailor services for each individual to meet the needs of our beloved Camas, WA residents. We also coordinate post-acute care pathways for joint replacement, spine, and cardiopulmonary recovery, ensuring continuity with your providers in Clark County. 

Leadership Team

Our Licensed Nursing Home Administrator (LNHA) directs all of Lacamas Creek Post Acute’s facility operations, staffing, services, and programs. This leadership team also includes our Medical Director, who coordinates medical services according to policies and procedures, as well as our Director of Nursing, Rehabilitation Director, Social Services Director, and Environmental Services Director at our skilled nursing facility in Camas, WA.

Nursing / Clinical Staff

Led by the Director of Nursing, our nursing staff delivers care and clinical services according to quality standards. Registered Nurses (RNs) assess, plan, provide, and document nursing care. They oversee Certified Nursing Assistants (CNAs), who assist patients with activities of daily living including administering medications, eating, bathing, dressing, and transferring. Around-the-clock services include 24 hour nursing care for prompt, compassionate support, along with IV therapy, pain management, and close monitoring for complex conditions.

Therapy / Rehabilitation

Our licensed Physical Therapists (PTs), Speech Therapists (STs), and Occupational Therapists (OTs) create and implement therapy plans according to individual needs. Under the guidance of therapists, Restorative Nurse Assistants (RNAs) assist with regular activities to improve mobility and support recovery. This team is led by our Rehabilitation Director, who guides therapy and rehabilitation strategies, operations, and staff within our Camas WA rehabilitation center. Programs include joint replacement and orthopedic rehab, spinal surgery rehab, stroke rehab, cardiac rehab, pulmonary care, fall prevention and balance programs, and cancer recovery.

Social Services & Activities

The Social Services Director leads and plans programs that address the mental, social, and emotional needs of patients and families, and oversees Social Workers. Social Workers identify and coordinate individual services to maximize independence, dignity, and quality of life, including guidance for those seeking memory care near Vancouver, WA, resources at Lacamas Creek Post Acute, and support with discharge planning and community resources in the City of Camas and Clark County areas.

Dietary Services

Registered Dietitians (RDs) oversee meal planning and presentation to ensure nutritious, tasty meals that fit each patient’s needs, supporting recovery and wellness in our post acute care facility. Therapeutic diets are coordinated with therapy goals and clinical care pathways.

Environmental Services

The Environmental Services team oversees facility maintenance, laundry, general housekeeping, and cleanliness according to health and safety standards to enhance comfort across our skilled nursing facility in Camas, WA. Amenities include engaging activities, an on-site beauty parlor, and convenient in-house therapy.
